Please complete all mandatory forms for each child that attends Memorial School.
Please return all of these forms with your child during 1st week of school.
*Kindergarten parents - please note, only preschool students moving to kindergarten need to complete the Emergency Dismissal, Medical Emergency forms and the Military Parent Questionnaire. New registrations completed these forms as part of the enrollment packet. (However, all families are require to review the Student Handbook then print, sign and return the Student Handbook Signature Page.)
All students are assigned to a bus based on their home address. If your child will not ride his/her assigned bus home, has a prearranged play date, will be a parent pick-up at the end of the day, needs to be dismissed for any reason, a note from a parent is required. For your convenience, a Change of Transportation form is attached.
PLEASE NOTE, TRANSPORTATION NOTES FROM THE PREVIOUS SCHOOL YEAR DO NOT CARRY OVER. Please send transportation changes in with your child in the morning. The office staff will note the change and issue a pass based on the information received.
As a reminder, anyone who dismisses a student during school hours is required to provide a valid drivers license or government issued ID.
