IMPORTANT - ACTION REQUIRED: CHROMEBOOK USER AGREEMENT FORM 2022-2023

To clarify basic information related to the use and care of Chromebooks and to support the rules and procedures of SRSD technology, please review and complete this SRSD Grades 3-12 Chromebook User Agreement 2022-2023. This form contains links to the SRSD Chromebook Care & Use Guide and the SRSD Student Computer & Internet Rules: Acceptable Use Policy, as well as information about the optional Chromebook Protection Plan. NOTE: If you have multiple students in grades 3-12 you will need to fill out a form for each student (sorry about that!)

Chromebooks will be issued only to students who have completed the Chromebook User Agreement Form (linked above) and the following two mandatory documents: 

• Parent-Student Handbook Acknowledgment Form (these are school-specific and you may access Memorial School's Student Handbook here and the mandatory Student Handbook Acknowledgment Form here).
• Internet Access Permission Form (this is one of the back-to-school forms in Alma, so if you have already completed those forms, you have fulfilled this requirement).
